I’m so excited to have you share your story—it’s going to inspire so many people. This process is designed to help us focus on the most meaningful parts of your journey and ensure we capture it in the best way possible. Don’t worry about perfect answers—just share from the heart.

This questionnaire will help you share your story in a way that feels natural and impactful. Focus on the key moments and lessons learned—no need for perfection, just authenticity. You’ll also have the option to upload photos or visuals to enhance your story. Once completed, you’ll receive an email with a link to your responses. Take your time, and remember, this process is as much for you as it is for us. Let’s get started!

Before Addiction

Let's start by setting the stage.  This is all about who you were before addiction became part of your life/family.  Think about the people, moments, and emotions that shaped you during this time.

Now let's dive into the heart of your story.  This is where we explore what life was like during your addiction- the challenges, the people around you, and the moments that stand out.

This is where everything changes-- the moment that shifted things for you.  Let's focus on what made this moment different and how it set you on a new path)

Section 5: Visuals to Bring Your Story to Life

Photos, short videos, and other visueals can add so much depth and emotion to your story.  They help people connect with your joruney on a more personal level and make your story even more impactful.  If you're comfortable, I'd love to include some meaningful visuals along side your interview.  

By uploading any images, videos, or other visuals, you’re giving permission for them to be used in the video and any related materials or clips created from it.That said, if you’re not comfortable sharing visuals, that’s totally okay—there’s no pressure at all.
